To apply for the Individual number card you must go the city office where you are a resident of. You can ask them to help you to fill-out the form to get the plastic version. They will probably tell you that you do not need it because your number card should really only be for government uses. Therefore only having the paper version would be enough.
I too have only the paper version and still a little undecided on if I should get the plastic one. I received a packet of information on how to do it from the city hall in English when I first moved to where I am now and that was nice.

Do change that paper slip for the card - these days they even offer printing your name in braille as an extra!My biggest benefit of having it so far is that I can now print my 住民票 in any combini, withouth visiting the office hall everytime. I also use it as an ID for registering in mercari, furusato nozei and online banks - since its the mostly accepted document and doesnt require further identity verifications.
I only have the paper mynumber card/slip (because it doesn't expire or need to be renewed like the plastic version of the mynumber card). Are there any pros/cons to actually getting the plastic card version of mynumber?
Hi! Good question! The paper form that you have is just a notification card without an IC and photo which could make your life a little harder in terms of getting official documents online. If you aren't concerned about the government's big brother approach then apply for one.
